DESCRIPTION:This comprehensive 6-week series will provide the LTC/AL team with detailed fall management program components to effectively reduce fall risk\, occurrences and possible fall-related injuries. Topics will include: *QAPI systems to drive improved safety*Keys to better fall risk assessments*Insights into effective Root Cause Analysis*Intervention selection that targets individual resident risk factors*Developing comprehensive fall management care plans*Elevating staff engagement in fall prevention efforts. \nTarget Audience: Nursing Leadership\, Nursing Home Administrators\, Department Heads\, Owner/Operators\, QAPI team\, IDT Fall Investigation Members\, Therapy\, Restorative Nurses\, Risk Management CEU: This program will be NAB approved for 1 contact hour for each session attended live.\nJune 17 - Considering all the Evidence: Assessing Fall Risk Critically Explore how clinical changes\, medication\, environment\, and systems of care interact to increase fall risk\, and how gaps between assessment\, care planning\, and implementation contribute to adverse outcomes. June 24 - Getting to the "Root" of the Fall with Root Cause Analysis Effective fall management interventions cannot be implemented if the root cause of the fall has not been identified. This session will focus on completion of a detailed root cause analysis that will guide the facility in determining the underlying cause of falls. July 1 - Addressing Fall Root Causes with Targeted Interventions Falls are a frequent challenge in skilled nursing settings. This presentation focuses on moving beyond surface-level fall reviews to meaningful root cause analysis and effective resident-centered interventions. July 8 - Engaging the IDT Troops in Fall Management This webinar will identify strategies for assigning IDT roles/duties for fall management. Sample tools will be reviewed to assist the team members with their assigned roles\, as well as insights for conducting IDT meetings for successful identification\, implementation and monitoring of fall management interventions. July 15 - Care Planning Strategies for Achieving Success Fall Management Interventions cannot be effective if they are not addressed on the care plan\, implemented consistently\, and monitored/revised as needed. This webinar will focus on successful care plan strategies for fall management. July 22 - QAPI Tactics for Sustaining Positive Outcomes This presentation examines how an effective QAPI program can be used to reduce fall risk\, prevent repeat falls\, and improve resident outcomes through data-driven decision making.
